Treatment is not a single surgery, but a sequential protocol designed to support the child's natural growth and development. Every intervention has an optimal timeframe in the patient's life to maximize aesthetic, speech, and functional results.
Ideal Surgical Treatment Timeline
1. Cleft Lip Repair (Cheiloplasty)
When performed: Between 3 and 6 months of age.
Goal: Restore muscle continuity of the lip, improve nasal symmetry, and create a balanced facial appearance. This first intervention is an important emotional milestone for the family.
2. Cleft Palate Repair (Palatoplastia)
When performed: Between 9 and 12 months of age.
Goal: Close the connection between the oral cavity and nasal cavity. Performing this surgery before the child develops speech habits is crucial for proper sound production and ear infection prevention.
3. Alveolar Bone Graft
When performed: Between 8 and 11 years of age.
Goal: Before permanent canines erupt, a small bone graft (usually from the hip) is placed in the gum cleft (alveolus). This provides bone support for permanent teeth and stabilizes the maxilla.
4. Orthognathic Surgery and Definitive Rhinoplasty
When performed: Between 16 and 21 years of age (when facial growth finishes).
Goal: Many cleft palate patients develop a smaller upper jaw compared to the lower jaw. Orthognathic surgery repositions the jaw for an optimal bite, followed by rhinoplasty to refine nasal aesthetics.
